New Patient Information

When your doctor refers you for physical therapy, a prescription is written. Most insurance companies require this prescription to be on file with the therapy office so please bring it with you to your first appointment. Some insurers may require you to have authorization from them before you begin your PT. If necessary, we will obtain this authorization for you once we have your insurance information.

Some insurance companies designate preferred providers and you will receive a higher benefit when you see a Preferred Provider in their network than you would if you saw an out-of-network provider.

If your insurance provider is not on this list, please contact them to check on your level of benefits.

If this is your first visit to our clinic, we will ask you to complete a New Patient Registration Form, a Patient Medical History, HIPPA form and sign the Patient Financial Responsibility and Assignment of Benefits in order to bill your insurance for you
